Bristol After Stroke is looking for enthusiastic, skilled and motivated individuals to be part of an exciting new service supporting stroke affected people adapt to life after stroke.  We have a number of full and part time roles available. 

This is a home-based role, which will require travel across working patches in either Bristol or South Gloucestershire. 
 
You will be working in a team of Stroke Key workers to offer support and six-month reviews to stroke patients in the region. You will be supporting the development of this new and exciting model, trialing new ways of working with the organisation and other health professionals in the stroke pathway.  

You will need to have excellent interpersonal skills, knowledge of wider community support and a passion for supporting people rebuild their lives after stroke.
