Post: Stroke Support Coordinator

Salary  £25,623 per annum pro rata

Contract: Permanent

Hours: 21 hours per week

Location: Homebased - will include visits to people’s homes, mainly in South Glos, but the service covers Bristol & South Glos. Occasional attendance at the office in Hartcliffe.

Bristol After Stroke is looking for enthusiastic, skilled and motivated individual to be part of a pioneering service supporting stroke-affected people adapt to life after stroke.  

This is a home-based role, which will require travel across South Gloucestershire. 

You will be working in a team of Stroke Support Coordinators to offer support and six-month reviews to stroke patients in the region. 

You will need to have excellent interpersonal skills, knowledge of wider community support and a passion for supporting people to rebuild their lives after stroke.  

You will be joining a long-established local charity with a close and committed team.
